9eddd42347 fix win32 (non-glut) cmake build.
add partId/index to btCollisionObjectWrapper to be able to recognize recursive shape indices (a btCompoundShape with a btGimpactShape child shape etc)

73b217fb07 Apple contribution for OSX SSE and iOS NEON optimizations unit tests, thanks to Jordan Hubbard, Ian Ollmann and Hristo Hristov.
For OSX:
cd build
./premake_osx xcode4
for iOS:
cd build
./ios_build.sh
./ios_run.sh

Also integrated the branches/StackAllocation to make it easier to multi-thread collision detection in the near future. It avoids changing the btCollisionObject while performing collision detection.

As this is a large patch, some stuff might be temporarily broken, I'll keep an eye out on issues.

96d73d642d fixes in debug drawer: always use rgba color with each component in range [0..1]
use sphere/box rendering of debug drawer

2ef7c1a457 Fixes / improvements in soft body:
avoid blow-up due to improper mass calculation for fixed nodes (happened when using clusters)
allow to create collision clusters for each tetrahedron or triangle, when using btSoftBody::generateClusters(0)
tweak soft body demos a bit, only draw debug wireframe if necessary

e590f4a721 fixed some outstanding issues with Bullet soft bodies:
1) re-allocate a pool allocator, if one of the soft body collision algorithms is larger than max pool element size
2) manage child shapes properly, and call RemoveReferences on the m_sparsesdf

682a0a1b90 fixed btSoftBodyConcaveCollisionAlgorithm, wrong bounding box transformation
added vertex welding option for btTriangleMesh (brute-force slow)
reject appendFace for some degenerate triangles (all 3 vertices/nodes need to be different)
add setVelocity method for btSoftBody

8f3e9603f3 + fixed issue related to temporary/root collision shape in btCollisionObject
+ normalize plane normal input for btStaticPlaneShape
+ fixed issue related to swapped collision detectors (SphereTriangleDetector in particular)

f8e5481612 Added method to collect all persistent contact manifolds, given a btBroadphasePair (collision algorithm)
