22fb7d5c1e improve rolling friction using anisotropic direction, to avoid resting in an instable position
(for implicit capsule, cylinder and cone shape)

7eebb79ced implemented rolling friction, using a contact constraint. Useful to get rolling spheres to rest, even on a slightly sloped plane.

Fixes in FractureDemo (mouse picking constraint needs to be removed, otherwise constraint solver crashes/asserts)

9612561113 add btDiscreteDynamicsWorld::createPredictiveContacts after discrete collision detection and add temporary contact manifolds, for the constraint solver.
This should improve 'ccd' handling when using world->getDispatchInfo().m_useContinuous = true;body->setCcdSquareMotionThreshold(...); body->setCcdSquareMotionThreshold(...)
shoot smaller boxes (test)
use yellow instead of orange for contact point normals
tweak default erp and erp2 values, now split impulse is on by default (need to check it)

2f1014268b + Internal improvements for collision shapes
1) add AabbCaching versions of btPolyhedralConvexShape and btMultiSphereShape (this speeds up btMultiSphereShape 'getAabb', and reduces size of btBoxShape)
2) btCylinderShape doesn't derive from btBoxShape anymore
+ Minor fixes in drawing for btMultiSphereShape, btBoxShape.
+ Don't re-generate btDebugFont every frame
+ Disabled velocity prediction for btDbvtBroadphase. Previous default can be restored using btDbvtBroadphase->setVelocityPrediction(1./2.);

42ece54556 applied patch that allows to enable multithreading mode for CcdPhysicsDemo,
Still requires manually adding BulletMultiThreaded library dependency (we won't enable it by default just yet)

4894f24c9a Fixes for FPU exceptions, thanks to Phil Knight for reporting and John McCutchan for fix/workarounds.

dcd57f333b + improved KinematicCharacterController
+ improved btSubsimplexConvexCast, btContinuousConvexCollision and btGjkConvexCast to support configuration that start in touching/penetration, required for 'sliding'.

56c69bc42e - stop threads when exiting demo that uses multi threading
- improved friction model for parallel solver (align the friction direction with projected velocity, unless the projection is close to zero (only then use 2 arbitrary axis orthogonal to contact normal)

a101719687 - removed warnings in GLUI, thanks Jorrit Tyberghein for the patch!
- removed memory leaks from BasicDemo and Bullet library (other demos needs to be cleaned up!)
- added memory leak debugging functionality in btAlignedAlloc.h: #define BT_DEBUG_MEMORY_ALLOCATIONS

31f1ba8e47 Added support for IBM CELL SDK (Playstation 3 Linux, CellBlade) SPU Libspe2 compilation/linking. It runs but gives a bus-error, so some DMA alignment issue is still there.
This is interesting, because the same code runs fine on Playstation 3 with the Game SDK.

ffcdb0f93d BulletMultiThreaded needs to refreshContactPoint for contact manifold (this has been moved to the collision detector, instead of constraint solver)
BulletMultiThreaded support function for box shouldn't compensate for the collision margin anymore

eff4fe8ec8 more work on hashed pairmanager. growing doesn't work yet, so need to allocate enough room for the overlapping pairs in advance.
boxbox reports contact point in B, rather then average point
box, cylinder use halfextents corrected for scaling and margin. made the margin in this halfextents explicit in the 'getHalfExtentsWithMargin' and 'getHalfExtentsWithoutMargin'
integrated changed for ODE quickstep solver
replaced inline with SIMD_FORCE_INLINE
some minor optimizations in the btSequentialImpulseConstraintSolver

added cone drawing (for X,Y,Z cones)

b054f375bc - Added serialization to btBvhTriangleMeshShape/btOptimizedBvh. See ConcaveDemo for example usage.
- added bt32BitAxisSweep3, which co-exists without recompilation, using template class. This broadphase is recommended for large worlds with many objects (> 16384), until btMultiSwap is finished.
- Fixed some recent issues in Bullet 2.57 related to compound (thanks Proctoid) and memory allocations

87df3d0f32 Based on feedback from another professional game company, there are several improvements, including some API change...
Some dynamic memory allocations have been replace by pool allocation or stack allocations.
quantized aabb versus quantized aabb overlap check is made branch-free (helps a lot on consoles PS3/XBox 360)
Collision algorithms are now created through a new btDefaultCollisionConfiguration, to decouple dependency (this is the API change):
Example:
	btDefaultCollisionConfiguration* collisionConfiguration = new btDefaultCollisionConfiguration();
	m_dispatcher = new	btCollisionDispatcher(collisionConfiguration);

1f7646f72a Refactoring to enable multi SAP broadphase.
This was already planned, and Pierre Terdiman recent thread motivated me to decouple the paircache from the sweep and prune.
