79989a8d3d Force spuNodeCallback to be 16-byte aligned, so that we can use SIMD. Related to Issue 657
Recompute the quantization values in case the maximum/minimum extends roundtrip using quantize/unQuantize are out-of-range

73b217fb07 Apple contribution for OSX SSE and iOS NEON optimizations unit tests, thanks to Jordan Hubbard, Ian Ollmann and Hristo Hristov.
For OSX:

Also integrated the branches/StackAllocation to make it easier to multi-thread collision detection in the near future. It avoids changing the btCollisionObject while performing collision detection.

As this is a large patch, some stuff might be temporarily broken, I'll keep an eye out on issues.

8fbe399ea4 Make .bullet serialization mode robust:
Deal with broken DNA serialization. Bullet 2.76 release revision 2035 - 2046 was broken, and this includes the Maya Dynamica plugin.
Added some workaround to deal with the broken .bullet files, instead of crashing.

9acb3d1805 Added some improvements on Bullet serialization:
Introduced generated unique id's, instead of the pointer address to avoid 64bit->32bit truncation issues
Use serializer->getUniquePointer instead of directly using a pointer

moved  ChunkUtils::getNextBlock to bFile::getNextBlock.
Moved 'TEST_BACKWARD_FORWARD_COMPATIBILITY' to bDNA.cpp. Enable the define for further testing of .bullet format
Removed duplicate definitions and use the one in LinearMath/btSerialization.h

41e9115bca Add support to serialize btOptimizedBvh/btQuantizedBvh for a btBvhTriangleMeshShape (using the new btSerializer). This is a new implementation, with full cross-platform support.
So it is different from the in-place method (btQuantizedBvh::serializeInPlace/deserializeInPlace).

It is also possible to serialize/deserialize just the bvh, using the btSerializer (needs some code snippet/helper)

d4c3633405 Get rid of btSolverBody and use btRigidBody directly. btSolverBody didn't improve performance after all, due to random-access
Tweak the BenchmarkDemo a bit: 

1) disable deactivation in graphical mode
2) add some settings that increase performance in the BenchmarkDemo2 (1000 stack) from 35ms to 15ms on this quad core (at the cost of a bit of quality)

bb8d1b11df Add btBroadphaseInterface::aabbTest. btDbvtBroadphase and btAxisSweep3 should perform well, as long as the raycast accelerator is enabled.
This should fix the btCollisionWorld::contactTest(btCollisionObject* colObj, ContactResultCallback& resultCallback);

c6df06c6a1 Added btDbvtBroadphase::setAabbForceUpdate to always update the aabb (skip optimizations), to address this issue (helps with occlusion culling)

c26f5e4b6e apply patch to allow soft body rayTest. Minor change in patch: test for current hitfraction, so only report hits closer than current closest hit fraction.

ac3d52d81b bugfix in aabb expansion, causing the convexSweepTest to fail in certain cases
(when the AABB is not symmetric around the origin)

40c73f327c + CMake build system fix under Windows: don't define _WINDOWS to allow Glut console demo to build properly
+ Allow user to enable useConvexConservativeDistanceUtil . Use dynamicsWorld->getDispatchInfo().m_useConvexConservativeDistanceUtil = true;
(see Demos/Benchmarks/Benchmark4 (convex objects falling down)
+ Fix for plane drawing (just wire-frame)
+ Gimpact: use collision margin of 0.07 for demo (because BULLET_TRIANGLE_COLLISION is used)
+ replace dot,cross,distance,angle,triple in btVector3 by btDot, btCross,btDistance,btAngle,btDistance to avoid naming conflicts
+ Some fixes in GJK penetration depth normal direction (broken in a previous commit)
+ fix in calculateDiffAxisAngleQuaternion to make ConvexConservativeDistanceUtil work properly
+ allow debug drawing to debug btContinuousConvexCollision
+ add comment/warning that btTriangleMesh::findOrAddVertex is an internal method, users should use addTriangle instead

2f1014268b + Internal improvements for collision shapes
1) add AabbCaching versions of btPolyhedralConvexShape and btMultiSphereShape (this speeds up btMultiSphereShape 'getAabb', and reduces size of btBoxShape)
2) btCylinderShape doesn't derive from btBoxShape anymore
+ Minor fixes in drawing for btMultiSphereShape, btBoxShape.
+ Don't re-generate btDebugFont every frame
+ Disabled velocity prediction for btDbvtBroadphase. Previous default can be restored using btDbvtBroadphase->setVelocityPrediction(1./2.);

7a2a98078a Fixes for broadphase/paircache determinism.
Revert definition for ATTRIBUTE_ALIGNED16, and try to force sizeof(btSolverConstraint) by using unions with btScalar, for non-btScalar data types.
Use btAssert and not assert.
Don't access btAlignedObjectArray elements, for zero sets

a8ec916af0 Added Height Field Fluid Demo to Bullet. All code stored in the Demos/HeightFieldFluidDemo directory for now.
Please see HfFluidDemo.cpp for examples of how to use the height field fluid along with buoyant collision shapes.

The implementation is still lacking in my ways:
1) Need to complete more collision algorithms for buoyant collision shapes
2) Support compound buoyant shapes
3) The buoyancy model isn't that great
4) Fluid volume can be lost over time

6db1b932fd Make the btDbvtBroadphase deterministic/reproducable, by sorting the btBroadphasePair using unique ID, instead of proxy address.
The btAxisSweep3 didn't suffer from this issue, because proxies were always allocated in-order, as part of an array.

50344c4a23 Add support for broadphase acceleration of convex cast (re-use rayTest implementation with an added aabb min/max, zero for rays)
Add Concave Convexcast demo back in AllBulletDemos, and tweaked it a bit.
Fix view frustum of ForkLiftDemo (caused picking to fail)
Removed innerloop profiling for ray and convex casts, it hurts performance.
Set default #aabb's in CDTestFramework to 8192
